        * cciBindings2.c (MoCCITerminateAllConnections): Added code taken
        from MoCCISendOutput to remove all the elements in the
        listOfSendOutput

        TESTING:
          case 1: Server closes connection
            CCI Client is setup to receive data from Mosaic via
              MCCISendOutput.
            Server closes connection via CCI dialog in File menu.

            Verify that Mosaic detects the closed connection and redirects
            urls back to browser.

          case 2: Client dies while SendOutput is enabled
            CCI Client is setup to receive data from Mosaic via
              MCCISendOutput.
            Client dies with SendOutput enabled

            Verify that Mosaic detects the closed connection and redirects
            urls back to browser.
            MCCISendOutputStop  // send all urls to browser
            MCCIGet             // get a url (to browser)

          verify that the URL is gotten to both the CCI client program and
          the browser.  Verify that Mosaic does not get confused about the
          state of SendOutput
